Professional Annual Reports
ALL REPORTS SHOULD BE SUBMITTED TO REPORTS@ALPHACHISIGMA.ORG BY MAY 15 EACH YEAR.

This document is updated by the National Office to indicate which reports have been received from professional chapters. View the document to see what reports are outstanding or when the last report was submitted.
Annual Activity Report Guide
This form details activities held by each professional chapter and should be submitted with all of the annual reports each year.
Officer List
Complete this form immediately following officer elections or if any position is filled by a new member.
Sample Register
Each year, professional chapters must submit an updated member list. The National Office will send a member list to the Professional President and Secretary in February to update. This document is a sample of what the document should look like when submitted.
Professional Charter, Ritual, and Regalia Inspection (PCRRI) Form
Each year, chapters should do an inspection of their ritual and regalia. This form helps chapters confirm that all items are in good condition and in working order.

DISTRICT COUNSELOR COLLEGIATE CHAPTER VISIT REPORT
This document allows District Counselors to report on the status of collegiate chapters and groups after the DC visits. This is sent to the Grand Collegiate Alchemist to review as well.
District Counselor Manual of Procedure
The Manual of Procedure for the District Counselor provides insight into the position, as well as includes duties, reports and deadlines, and other important information for the Brother serving as a District Counselor.
Reporting and Recording Pledges Guide
The Reporting and Recording Pledges Guide is a great insight into how the entire pledge to an initiated member process works from the Fraternity side. This includes links to the various forms, screenshots to show what report and reminders are sent throughout the semester, etc. This can be helpful for District Counselors, Grand Collegiate Alchemists, and chapters themselves to know what to expect through the pledge reporting and recording process.
