PLEDGE RECORDING PROGRESS LOG
This document is updated by the National Office to include which chapters are having initiations, receipt of Pledge Report Forms, payment of fees, and initiation approvals.

The Pledge Interest Form was created to help track interested candidates and to collect their contact information for the Pledge Report Form if they do go through induction.

Physical sourcebooks are no longer shipped. Please direct pledges to download a digital copy of their sourcebook online.
PLEDGE REPORT FORM
This form should be completed and submitted immediately following the pinning or induction ceremony. This indicates to the National Office that your chapter plans to hold an initiation and who will be initiated. Faculty members and professional candidates going through initiation should also be included on this form. As of Fall 2019, this form is now electronic. Please make sure to follow the link below – do not submit the old PDF/Word file.
DEPLEDGE REPORT FORM
This form should be completed to report any pledges who will not be continuing on to initiation.
PROFESSIONAL CANDIDATE FOR INITIATION FORM
Submit this form when reporting professional (non-faculty) pledges, and include them on the Pledge Report Form. Remember that you will need permission from the Supreme Council to initiate anyone who is not a faculty member.

STAR AWARD CHECKLIST
Use this tool to help track your chapter’s progress towards a one, two or three-star award.
ANNUAL CHAPTER REPORT
This report advises the Fraternity about the chapter’s overall status. This will include initiations, PBI ceremonies, chapter activities, chapter publications, chapter social media & website, etc. The format of this form is *NEW* as of 3/18/24 and is completed online.
CHAPTER FINANCES
All chapters must perform an audit and complete the Chapter Audit/Statement of Finances form by May 15th of each year.
FILING THE E-POSTCARD WITH THE IRS
- If your chapter has gross receipts OVER $50,000, you are required to submit Form 990 or 990-EZ with the IRS. The Form 990 or 990EZ is due each year by the 15th day of the fifth month after the close of your tax year.
- Most chapters have gross receipts of $50,000 or less and therefore, must file the 990N e-Postcard. The 990N e-Postcard is due each year by the 15th day of the fifth month after the close of your tax year.
For Alpha Chi Sigma, our tax year ends on Dec. 31, so your 990N e-Postcard (or Form 990/990EZ) is due by May 15 each year. You cannot file the e-Postcard until after your tax year ends.

**The National Office will send each chapter instructions on how to complete the e-Postcard prior to the May 15 deadline.
CHAPTER PLAN
Chapters may use the fill-in form and/or the sample calendar to help plan the chapter’s annual activities, ceremonies, and deadlines. Chapters may also submit a calendar as the Chapter Plan, or they may type out a list of dates and events.
CHARTER, RITUAL, AND REGALIA INSPECTION (CRRI) FORM
Each year, chapters should do an inspection of their ritual and regalia. This form helps chapters confirm that all items are in good condition and in working order.
HEALTH & SAFETY TRAINING
Each chapter must complete Health & Safety Training annually. Health & Safety Training will be arranged through and/or conducted by your District Counselor. The District Counselor will report to the National Office once the training is complete. Please reach out to your District Counselor for more information and to schedule your Chapter’s training session.
In addition to required annual training on the Alpha Chi Sigma Health and Safety Policy, chapters are encouraged to attend additional training sessions related to health and safety offered by their institution. Please use the Additional H&S Training Form to report those sessions. Qualifying events count towards the 2 and 3 star awards.

Complete this form immediately following chapter officer elections or by May 15 (if there was no change in officers) each year.
PROFESSIONAL BRANCH INDUCTION (PBI) FORM
This form has several purposes. To report your graduates, to request professional pins for your graduates, and to notify the National Offices of a PIC or PBI ceremony. This form is required for a 2-star or 3-star award. As such, the form may also be used to indicate that your chapter has no graduates, so that the chapter may still be eligible for a 2-star or 3-star award.
- If hosting a PBI or PIC, please submit at least two weeks before the ceremony to allow the National Office enough time to process and send professional pins for each member listed on the form.
- If no ceremony is held, this form should be submitted by May 15 to indicate any members moving to the professional branch at the end of that school year.
- If your chapter has no graduates, this form should be submitted by May 15 indicating that you have no graduates.
